To Parents/Carers of students with Asthma and/or Allergies, if you have not already done so this year, please provide the school with an action plan signed by a doctor.
When getting an Asthma Plan from your doctor please ask them to use the correct form. Asthma Australia provide a form that has all the required details that we need, they can go to the website and individualise the form online before printing or they can email the form to the school.
Parents of all students with these medical conditions need to arrange an interview with one of the school’s First Aid Officers. These interviews need to happen once a year.
